I had a very positive experience with the care provided at this nursing home. The staff was consistently kind, attentive, and respectful, creating a warm and supportive environment. It was clear that they genuinely care about the residents and take the time to get to know each person. The therapy team was excellent—knowledgeable, patient, and encouraging throughout the recovery process. They worked closely with me to build strength and mobility, always making sure I felt safe and comfortable during sessions. The facility itself was clean, well-maintained, and organized. The communication between staff, residents, and families was also very reassuring and helped build trust. Overall, I’m very grateful for the care and support I received here. I would confidently recommend this nursing home to anyone looking for a place that provides both quality care and compassion.

My husband was there for eight days. It was the worst place to have to send someone you care for. The nurses staff was very uncaring and unhelpful. In those eight days, not once did he get any kind of a bath. Never had anything cold to drink, they said they didn’t have pitchers to put any ice or water in. His call button most of the time was pinned to the bed where he couldn’t reach it. The bed was broken when he first arrived. He ended up with a roommate. His wheelchair was put right by the other patients bed, he couldn’t even see his own TV. I could go on and on. Just know if you can find another place to go, please, please do. My husband ended up passing away, and I blame 95% of his passing on the lack of care he received at this facility.

I am writing this letter with a full heart as I prepare to leave Louisville East Post Acute after three years and five months. I wanted to put into words what this place has meant to me, in hopes that it might encourage others who are considering coming here. I arrived in October of 2022 in very difficult circumstances. I had survived cancer in 2019, during which a good portion of my left thigh was removed. Although I recovered, I was left unable to stand for long without falling. I was also carrying significant weight — nearly 278 pounds — and I was deep in grief following the loss of my husband. When I first arrived, I could not get out of bed on my own and I was incontinent. I want to be honest about that because I want people to understand how far I have come. The therapy department at Louisville East is simply the best I have ever experienced, and I tried many before coming here. Diana, my physical therapist, practices what she calls “hard love.” If I said I can’t, she said yes you can — and that is exactly what I needed. She would not let me give up on myself. Tony, my occupational therapist, had me lifting weights and riding the bicycle, doing something every single day. It got my mind off my grief and my depression and made me feel alive again. I was moving, and I was living. The nursing staff, the nurses’ aides, and my doctors all treated me with great kindness and professionalism throughout my stay. After about a year I was given a private room, which became my own little sanctuary. The food was wonderful, and the dietitian Kathy worked with me personally to arrange fruits, vegetables, and a managed diet that supported my health goals. Over the course of my time here I have lost approximately 70 pounds. My blood pressure, which had been dangerously unmanaged and may have contributed to my falls, is now well controlled through proper medication and diet. I had been using a walker before coming here and was still falling regularly — but through the care I received at Louisville East, I no longer fall. My walker remains my trusted companion, but I use it now with confidence and stability. The activities program also played a healing role that I cannot overstate. Under the leadership of activity director Angela, her assistant Jackie, and the wonderful volunteers who give their time to our community, the crafts, get-togethers, and special dinners created a sense of joy and belonging that supported my recovery in ways that go beyond medicine. I am now transitioning to independent living, something I could not have imagined when I first arrived here barely able to get out of bed. The moving trucks come today. Someone comes for me tomorrow. I leave with gratitude in my heart for every person at Louisville East who showed me kindness, gave me their knowledge and skill, and helped me find my way back to myself. My deepest thanks to the entire staff. Louisville East Post Acute is a truly healing place, and I would recommend it without hesitation to anyone in need of care. I recently spent 5 nights at this facility following ankle surgery. I was given a room on the 100 hall which overlooked the courtyard. During the tire stay nursing staff both licensed and CNA’s were very nice and attentive. Although food is almost always a concern at nursing facilities, mine was quite good , served hot and when asked for extra condiments they were always available. Therapy staff was helpful and helped me reach my short term goals. I slept well and meds arrived on time. Staff made a point of stopping to see me prior to discharge to wish me well. My spouse paid privately for the stay and it was well worth the money. If I need to get some short term rehab, again I would definitely recommend LEPA!!!!!

If you care about your loved one, I strongly advise against placing them in this facility. Our experience was deeply troubling and raised serious concerns regarding the quality of care and patient safety. My significant other was initially placed on the Pathways unit, where the staff was exceptional. However, after her transfer to the 200 Unit on Louisville East, the level of neglect and unprofessional behavior became apparent. We frequently had to advocate for her to be changed every 2–3 hours, which is the expected standard of care. Instead, she often went four or more hours without assistance. On multiple occasions, either I or another family member had to turn off her feeding tube because staff failed to do so during changes, creating a risk for aspiration. Several aides were openly rude, dismissive, or acted in an intimidating manner when we requested basic care. These same individuals repeatedly attempted to make us leave the room while providing care, which we eventually refused due to concerns about her safety and proper feeding tube management. Fortunately, staff members from other units—who were not assigned to her—took it upon themselves to check on her. They consistently reported finding her soaked, confirming our concerns regarding ongoing neglect. Additional safety issues were present. Some aides intentionally turned off the air conditioning at night despite being fully aware that she could not tolerate heat, causing her blood pressure to rise to dangerous levels. While we understood that her roommate preferred warmer temperatures, we attempted to compromise with fans. However, the room became unbearably hot. Repeated requests to switch bed locations or move rooms were denied by management, despite clear evidence of the medical risks involved. The bathroom toilet remained broken for weeks. Although we notified staff numerous times, and were repeatedly told a work order had been submitted, no repairs were ever made. Urine remained in the toilet for an extended period, eventually leaking into the adjacent room. Despite this, her wheelchair continued to be stored in the same bathroom, even with urine on the floor. These issues appeared to extend beyond her unit. During time outside, other residents openly shared their own experiences of neglect and mistreatment. We also had to supply basic care items such as chucks, which frequently went missing or were taken for other residents, despite being clearly labeled with her name and room number. I also purchased an expensive waterproof wedge to keep her elevated during showers—an important medical accommodation—which also went missing, along with several other personal items. I am incredibly grateful that we were able to remove her from this facility as quickly as we did. The level of neglect and lack of accountability we witnessed was unacceptable and deeply distressing.

Not a negative comment about the facility except they should be more cognizant of the APRN they have on staff. Vicki Raizor, APRN, was probably one of the rudest healthcare providers I have ever spoken to. Simply asking her a question about a family member’s condition put her on the defensive. There’s usually a reason people are defensive and rude and afraid to answer simple questions about patients. Perhaps she is in the wrong field.

Please use caution before considering this facility for your loved ones. Pathways itself is a 5-star experience — the care is wonderful, and every employee from the CNAs to the nurses treat patients like family. Unfortunately, the 200s unit was a completely different experience for my sibling. On this floor, care was often refused. CNAs rarely checked on my sibling unless we hunted them down to ask for help. One CNA even told me that “Q2 turns are not a thing” after refusing assistance. The bathroom in her room was out of order for three weeks, with a toilet full of urine and water dripping. Every time we asked about repairs, we were dismissed. Many times I had to call the front desk after my family member went four hours without any CNA responding. Worse, some CNAs acted aggressively toward my family. They mocked and verbally attacked my parents when we asked for help. I often had to secretly ask CNAs from other units to check on my sister because she was being purposely neglected — and they told me she was consistently left dirty. Basic care was ignored. CNAs repeatedly forgot to pause her feeding tube, and we often had to step in to stop her from choking. Her belongings, even clearly labeled with her name and room number, were stolen weekly. Such as clothes, her chucks ($50 weekly missing), soaps, lotions, other misc items. This neglect was not an isolated incident but a repeated, ongoing problem on the 200s floor. Families deserve transparency and safe care for their loved ones.

Only familiar with the short-term rehab side of the building, not with the nursing home section. The rehab team is very helpful and informative. A few of the aides were very nice and kind to us

It’s a fair looking facility and the staff we talked to were nice overall. Therapists were very helpful. My SIL had been to a few other rehab buildings in the last year and they were awful compared to here. If we had to go through rehab again with her, we’d likely come here if given the chance.

Don’t let the pictures fool you. My sister there for therapy, very little improvement in her reason for being there. Very rude staff, my sister needed to be changed and I asked her when the last time she had been changed she stated that she had not been changed sine they got her up at 630 and it was 2 PM. My sister is one for UTIs was saturated and doubled briefed. Lazy so they don’t have to changed them. Nurses always talking down the aides, very out dated facility pictures are far more better than the facility. I would not recommend this facility at all. It is nasty

As a patient here I have to say the staff leaves you hours on end with a wet or dirty diaper and patients hurt themselves trying to get to the bathroom to clean themselves up. It’s awful. They don’t care. Number 1 excuse is “I don’t have you today” it’s pitiful. I wouldn’t put my mother in this place. Going forward if I ever need a place like this please don’t ever send me to one. Let me go be with Jesus before that happens. Their rehab is good but I can do that part as long as I didn’t have to come in contact with a CNA or nurse. Never again will I check myself into a place like this. Worse experience of my life.

Once again I am giving a review because I am not sure how someone can be RUDE to the elderly people at this place. The person at the front desk today was very rude to people older than her. Her name is Cathy. I felt sorry for the person she was rude to. I was walking out with something I need to take to my car and there was an elderly person who was waiting to go out they were pushing someone in a wheelchair and were trying to go out. Cathy was telling the person you have to sign out before you can leave and the person said my sister is going to do it. She signed us in and Cathy said you can’t go out yet because you are not signed out. The lady had a look on her face that was kinda heartbreaking to see. I don’t know how Cathy was so rude. I know she is not a nice person to most people anyway but I really was upset at how she was treating the other person. I was trying to get out my self and pushed the button by the door like I am used to doing the door didn’t open right away but I know how to get out and she was telling me I had to wait for the door to open which was neither here or there but she was refusing to press a button at the desk to let us out. The door did open not sure because I pushed it open or she finally pushed a button to let us out. I really felt sorry for the other people she was being RUDE to. I just don’t understand why she is that way. The other person just looked so sad. I don’t think Cathy deserves her job here. Like I am so happy my mom is leaving this place so I don’t have to put up with Cathy rudeness. I can kinda deal with her rudeness to me which she shouldn’t be but when she is RUDE to people older than her I can’t not get upset about it. That is not a good person to have working anywhere to be honest. I am just leaving this review to tell you all how rude she is and should have more respect for other people

*Mom stayed a week. *Very little improvement-yet positive change in her issue for being there. * I have never been told “it’s not my job” by so many staff people. Ever. Anywhere. *Vast differences in the front line care staff. We had several – off the charts great nurses and assistants-. But some left my Mother in soiled clothing in her bed after requesting help repeatedly with no response. *If you send someone you love there, be sure to be very proactive about their care.

I am a friend or relative of a current/past resident Tried to get my brother a water pitcher the stated they only get cups, then we was looking for a wash cloth they looked at my niece like she was asking for a million dollars they stated they don’t have access to them my niece well can I have one!! What is going on in this place!!hope he gets out of this place soon!

My husband was sent there for rehab to become stronger after a severe infection. While he was there he hurt his knee cause he was encouraged to peddle around on a wheelchair with no feet. His foot got stuck under chair and it cause him to twisted his knee. They never even looked at the knee he could not put any pressure on it. The pt people cam in to do therpy when he told them that he refused to do therpy. The had him transfered out of there back to hosptial. They called me and only said he seemed confused and agitated. Well at the hosptial had had 4 strokes. That the rehab did not notice. He shared a room but it was so damn hot in it. The roommate was about 120 pounds and my husband is over 300. The room mate bed was right next to ac that he did not like on and his bed was twice the size of his. There was one really nice nurse that only woored the weekends. The night nurses were very loud and acted like they were throwing a party. My first week there i had no problem with staff until he was injured. PLEASE DO NOT SEND A LOVE ONE HERE!!!!

My dad came for short term rehab and we especially love the staff. Everyone is so nice and helpful.
